Not everyone requires a surgical procedure to improve their signs of facial aging. Some patients may be too young and have early, evolving, and less extensive changes that are not so advanced as to require surgical intervention. Some may want to institute treatments early to try to maintain their appearance and forestall the appearance of the changes of aging. Others may be either too apprehensive to consider surgery, are not appropriate candidates for surgery because of significant medical problems, or are unable to take the necessary time off from work or their other responsibilities to adequately recover after surgery. Some may be patients who have already had surgery and are candidates for additional non-surgical procedures to address problems not amenable to surgery or to further enhance or compliment their surgical results.

As with any consultation for consideration of a surgical procedure, Dr. Gold will discuss your concerns and expectations during your consultation and will help you to understand and determine with you what procedures would be best for you to achieve your goals. The non-surgical ancillary procedures available through our office may be divided into several categories and include the following:


Light and Energy Based Treatments

Long Island IPL TreatmentIntense Pulsed Light (IPL) treatments can be used for improvement in the appearance, texture and tone of the skin by stimulating its deeper layers. A series of treatments over several weeks or even months may be necessary to see and feel the results, and while those treatments are often beneficial, they cannot replicate the results of surgery or more invasive procedures. Such treatments may also be used to improve areas of uneven or irregular (darker) pigmentation or "dyschromia", as well as the appearance of freckles or areas of sun damage, and prominent fine blood vessels.

It is a little known fact the humans have more hair follicles per square inch than most higher primates, including chimpanzees and gorillas. Some hair is coarse and dark, and some is so fine and pale that it isn't visible to the naked eye. When more prominent or darker, coarser hair appears in places you don't like, or expect, on the face, neck, abdomen, breasts, back, arms, or underarms, or, for women, prevents you from wearing high-cut bathing suit, shorts, or even a short skirt, it may be time to consider hair removal. Some types of lasers and other light based therapies can be used for effective hair removal on any part of the face or body in men or women, with this again requiring a series of treatments over time.

Skin Resurfacing

Long Island MicrodermabrasionMicrodermabrasion employees a hard jet of air containing fine microcrystals to remove the very most superficial layer of the skin, and is most effective in treating fine facial lines and superficial pigmentation changes, and can reduce the apparent size of your pores and improve the texture of your skin. There is no "down time" or healing period required, but a series of treatments will be necessary over time. Deeper facial wrinkles ("rhytids"), either etched in the skin from repeated muscle motion ("dynamic rhytids"), or present at rest from sun damage and aging ("static rhytids"), as well as facial scarring, either from trauma, acne, or even viral infections such a chicken pox, require a slightly more aggressive approach, and may also benefit from one of three modalities of treatments. Each can require one to two weeks for healing before patients can return comfortably to social interactions or work. Dermabrasion is the mechanical removal of the outer layers of the skin with a power-driven diamond wheel, smoothing out the surface irregularities. Laser resurfacing, utilizing an Erbium or Carbon Dioxide (CO2) laser or a blend of both, can similarly smooth surface irregularities and also can tighten the deeper layers of the skin. Chemical peels, can also be used for the same purpose. Superficial chemical peels (alpha hydroxyl or fruit acids, glycolic acid, salicylic acid) and deep chemical peels (trichloroacetic acid or "TCA", phenol) can be used separately or in combination to achieve varying depths of peel penetration and resurfacing depending upon the needs of the individual patient.

BOTOX® Injections

Long Island BOTOXInjections into the muscles of the face and neck, especially of the forehead and of the areas between the eyebrows and at the outer corners of the eyes, can temporarily inhibit or totally block muscle contraction. It can therefore either prevent the development of, or soften the appearance of, frown lines, "crows feet", and other lines that form as the result of facial expression. BOTOX® injection is a simple, non-surgical procedure with no downtime or recovery period necessary, but with results taking up to ten days to become apparent. The effects usually last from three to five months, and the injections can be repeated at that time when muscle motion starts to return in order to maintain the results.

Soft Tissue Fillers

Long Island Collagen Zyderm Zyplast Captique Hylaform Restylane Perlane Juvederm Radiesse Sculptra Fat InjectionsFacial creases and sometimes even fine lines can be filled, expression lines softened, and facial contours such as the lips and cheeks can augmented with a variety of facial "fillers". Dr. Gold has a long experience with all of the available "filling materials", including Collagen, Zyderm®, Zyplast®, Captique™, Hylaform®, Restylane®, Perlane®, Juvederm™, Radiesse®, Sculptra™, and your own fat. He will discuss with you the advantages and disadvantages of each filler and recommend the one or ones best suited to address your individual needs. The results of many fillers can often be seen immediately, although some require a series of injections and your body’s local reaction over time to produce their changes. All may occasionally be accompanied by some bruising which can last from several days to as much as several weeks according to the technique and the filler used.
